Apical Resection

Tooth extraction is not the only option in the presence of an abscess in the roots of the teeth. Root canal treatment is done first. Even if it is not enough, there is a chance of resection. Apical resection is the treatment of persistent root abscesses by direct intervention to the root of the tooth from the outside.

In cases where root canal treatment is insufficient and the patient's pain continues, apical resection is performed. In the area where the root of the tooth and abscess are located, a small window is opened over the jawbone and the abscess is cleaned. Apical Resection

On the x-ray, it appears as a black, rounded area at the root tip. Sometimes it manifests itself as an inflammation path (fistula) and swelling in the gingiva.
Treatment begins with the application of anesthesia. The gingival flap is removed by making an incision. Some of the bones near the root end of the teeth are removed. The root end of the tooth is cut and pulled with the support of burs. In root canal treatment, the roots of the teeth at the root end are carved a little, and every open part of the tooth is covered with a waterproof coating filling material. When the cyst is removed, the remaining space is filled with bone grafts if any fracture has occurred. Finally, the soft tissue is closed with the support of the sutures and the apical resection operation is completed.

Apical resection is not performed in unexplained root canal treatment failures. First, the cause of the failure must be determined. The positions of the anatomical formations, the short root of the tooth, especially if the teeth are already loose, will be a problem. In addition, it is not applied in case of systemic diseases.
Apical resection is generally applied to anterior teeth and premolars. However, it can also be successfully applied in cases where maxillary sinus and nerve relations with tooth roots are suitable and in posterior group teeth where it is easy to reach the root tip.
Although the duration of this process varies depending on the experience of the physician, the condition of the patient and the nature of the tooth to be made, it takes about half an hour.
  • Do not move your lip too much so that the stitches do not open
